The success of dental implants relies on the aesthetics and stability of surrounding tissues, influenced by factors like prosthetic connections. This trial compared peri-implant tissue behavior in 15 anterior implants with prefabricated titanium abutments (GT) and 15 with customized zirconia abutments (GZ). Evaluation occurred at four time points: the beginning of tissue conditioning (T0), end of tissue conditioning (T1), 7 days after final cementation (T2), and 6 months after final cementation (T3). Clinical parameters included Visible Plaque Index (VPI), Gingival Bleeding Index (GBI), Bleeding on Probing (BoP), Probing Depth (PD), Interdental Papilla Height/Width Ratio (PH/PW), Keratinized Mucosa Width (KM), Gingival Thickness (GTh), Gingival Phenotype (GP), Gingival Recession (GR), radiographic distance from the bone crest to the contact point, and Pink Esthetic Score (PES). Statistical analysis revealed no significant differences between groups for any clinical parameters at any time point (p>0.05). However, intragroup analysis showed a significant reduction in VPI between T1 and T2 only for GZ (p<0.05). Both groups showed a significant decrease in bone crest to contact point distance between T0 and T3 (GT: 1.67 mm to 1.47 mm; GZ: 1.70 mm to 1.47 mm) and an increase in PH/PW ratio (GT: 0.56 to 0.80; GZ: 0.70 to 0.83) (p<0.0001). PES significantly increased (GT: 9 to 12; GZ: 7 to 12) from T0 to T1 and remained high.
The study concludes that prosthetic abutment material choice does not significantly impact peri-implant tissue outcomes, allowing prioritization of cost-effectiveness, workflow, and treatment efficiency in implant rehabilitation in the aesthetic zone, ultimately enhancing patient results and satisfaction.

This study evaluated the diagnostic accuracy of a new self-reporting instrument for population screening of apical periodontitis (SRAP), using different image exams as the reference standard. A consecutive sample of 107 patients undergoing complete periapical radiographs and 123 individuals with cone-beam computed tomography (CBCT) were recruited. The number of teeth with apical periodontitis (AP) on image exams were recorded for each individual. SRAP was collected through an 8-item questionnaire. The validity of SRAP was determined based on the diagnosis of AP from image exams, calculated using standard methods. A logistic regression model was applied to SRAP scores to estimate the probability of AP occurrence for each SRAP score level. The mean age was 52.2 ± 15.5 years, with 65.6% females. The prevalence of AP was 57.9% (periapical radiography) and 64.2% (CBCT). The validity values of SRAP for periapical radiography and CBCT were, respectively: accuracy = 0.72, 0.74; sensitivity = 0.82, 0.75; specificity = 0.58, 0.73; efficiency = 0.70 and 0.74; + predictive value (PV) = 0.73, 0.83; - PV = 0.70, 0.61; + likelihood ratio (LR) = 1.95, 2.73; - LR = 0.31, 0.35; and area under the ROC curve = 0.75, 0.81 (p<0.05). Individuals with SRAP scores ≥4 had an almost 8-fold increased chance of presenting AP on CBCT images compared to those with lower SRAP scores (OR=7.87; 95%CI = 3.41 - 18.13). For each unit increase in the SRAP score, the likelihood of the patient presenting AP in the CBCT exam was almost doubled (OR=1.98; 95%CI = 11.51 - 2.58).
SRAP revealed to be an accurate population method for predicting individuals with a higher likelihood of presenting with AP, and may be a valuable tool for the epidemiological screening of AP and oral health surveillance.
